As we moved into the era of value-based care we engaged with ForestVue Healthcare Solutions to perform a comprehensive organizational capabilities assessment for population health analytics using their DataRevue offering. …Through a comprehensive survey process, ForestVue was able to quickly and efficiently identify actionable gaps in the people, infrastructure, process, and tools domains. While multiple areas were identified for attention, ForestVue was able to delineate eight high-priority opportunities that we could address immediately to move us forward on our journey to a robust program of population health analytics. This service was invaluable in that it provided us with an external, unbiased view of our existing analytics capabilities and available resources, as well as a clear roadmap of what we needed to do to move to the next level. As a result of their assessment, we were able to develop a small-scale internal data repository for blending data from disparate EHRs, replace numerous manual data extractions with automated reports, build and deploy a standardized set of operational dashboards, and implement the initial building blocks of an organizational data governance program. We have been following the ForestVue population health analytics journey for over two years. We began with the building of a comprehensive well-thought-out data infrastructure to support person-centric analytics in our first-ever implementation of an electronic medical record for the health plan. Through their DataReVue process, ForestVue provided us with a blueprint and support services resulting in a data model that supports advanced analytics for population assessment, risk prediction, and intervention monitoring. Using this infrastructure, we’ve been able to transition our analysts from cleaning and coding data to gleaning valuable insights resulting in cost savings and actionable insights.
